What happened
A hostname asks for Cloudflare Access in front, and the policies to apply are not configured.

What to do
Set CF_ACCESS_POLICY_IDS and CF_ACCESS_BYPASS_POLICY_ID on node/v1, or declare `access: false` to serve the hostname publicly.
